New clients may download and complete forms in advance of your first appoinmtment.  You will need to complete the following:

• Client Assessment
• Insurance Information
• Consent for treatment (or Minor Consent if the client is a minor)

Your signature on the Consent Form verifies that you have received and read the  Ephesians Statement of Policies and the Privacy Practices.
